2021 COVID Acknowledgement Form

 

All students who attend any in-person function at HCPS will be required to have the 2021 COVID Acknowledgement signed prior to attendance.  The following can be utilized, but a signed form either paper or online is required for attendance.

We will need to ensure that all students have a signed COVID Acknowledgement Form prior to attendance to in-person instruction.  If a family refuses to sign the form, the student will be required to receive instruction through virtual-classrooms only.  If a student arrives to school without a COVID Acknowledgement Form signed, the student must report to the school nurse to get a verbal acknowledgement by the parent over the phone for one day only.

 You can review the information from the Harford County Board of Education about the school system and countywide policies and rules, on the "For Parents" tab, under the "Parent Connection" link.  This information is important to you and your family.  You are responsible to be thoroughly familiar with all the information given under those tabs.

 NoteThis website does not include everything.  Your student teachers will give you further information about their classes.  Your student homeroom teacher, counselor, and administrator will provide other information through out the year.  If you cannot find answers to questions or concerns you may have, talk to your student teacher, counselor, or an administrator.  They are here to help you grow and enjoy your students stay at Havre de Grace Middle School.
